of State has recently enacted new procedures and forms for those applying
for passports (new and otherwise).
Some of the forms
in the following section, which are part of the department's new digital
document management program, must be completed online at the US Dept of
State website, Travel.State.Gov. Your information is secured and transmitted
directly to the US Department of State via their secure servers.
Our website will
conveniently open these secure forms for you to complete without having
to lose your place on our website. When you have finished your forms on
the secure server, you can simply close the window in which the form appears;
thus returning you to our site for final instructions for completing your
passport request(s).

is over 16 years of age and has never had a passport.

is over 16 years of age and has had a passport which was issued more
than fifteen (15) years ago.

is over 16 years of age who has had a passport which has been lost
or stolen (will also require form DS-64 in addition to DS-11).
» Applicant
is over 16 years of age and has had a passport which has been damaged
(will require written statement explaining the condition of passport
in addition to form DS-11).
» Applicant
is under 16 years of age and has had a passport.
» Applicant
is under 16 years of age and has never had a passport.
