Replace a Lost or Stolen U.S. Passport

If your valid U.S. passport has been lost or stolen, you should report it right away and apply for a new passport if you need to travel.

First: Was Your Passport Lost or Stolen?

This page is for travelers whose valid U.S. passport was lost or stolen.

Do not use this page for:

  • A damaged passport
  • An expired passport
  • A normal passport renewal
  • A passport name change

The U.S. Department of State says you should only report a passport lost or stolen if the physical passport is missing and it is still valid. Once reported lost or stolen, it cannot be used for travel, even if you find it later.

⚠️ Important: Reporting Your Passport Does Not Replace It

Reporting a lost or stolen passport cancels the missing passport, but it does not automatically give you a new one.

To get a replacement passport, you must apply for a new passport using Form DS-11.

How the Lost or Stolen Passport Process Works

Step 1 — Report the Passport Lost or Stolen

You can report a lost or stolen passport online, by mail, or in person when applying for a new passport. The Department of State says online reports usually cancel the passport within one business day.

Step 2 — Complete Form DS-11

A lost or stolen passport replacement requires Form DS-11. This means you must apply in person at a passport acceptance facility.

Step 3 — Include Lost/Stolen Details

On the DS-11 form, include where, when, and how the passport was lost or stolen. If you filed a police report, include a copy.

Step 4 — Visit a Passport Acceptance Facility

Bring your application, proof of citizenship, ID, passport photo, and supporting documents. The agent will seal your application package.

Step 5 — Mail the Sealed Envelope to Texas Tower

Do not open the sealed envelope. Mail it directly to Texas Tower Passport and Visa for expedited processing.

🚫 Do Not Use a Passport Reported Lost or Stolen

If you find your passport after reporting it lost or stolen, do not use it for travel.

Once the passport has been reported, it is canceled and is no longer valid for international travel.

⚠️ Important: Do Not Open the Sealed Envelope

Once the passport acceptance facility seals your application package, do not open it.

Opening the envelope can delay or void the application. Mail the sealed package directly to Texas Tower Passport and Visa.
