How the Name Change Process Works
Step 1 — Choose the Correct Form
Use DS-5504, DS-82, or DS-11 depending on your situation.
Step 2 — Gather Required Documents
You will need your current passport, proof of name change, passport photos, and Texas Tower documents.
Step 3 — Submit Your Application
Some applications can be mailed directly, while others must be completed in person at a passport acceptance facility.
Step 4 — Send Your Documents to Texas Tower
Mail your application and supporting documents for expedited processing.

⚠️ Important: Your Name Must Match Your Ticket
The name on your passport must match your airline ticket exactly.
If your ticket is booked under your new name but your passport still shows your old name, you may not be allowed to board your flight.
