Lost or Stolen Passport Replacement Steps Information
Complete and print the passport application online at the Department of State. Do not sign. Department of State's passport portal.
Print a copy of the unsigned DS-11 (passport form) This will be included with your Texas Tower order
Fill out DS-64, Statement for Lost or Stolen Passport and print one copy. Sign in black ink only. The original must be included in the sealed enveloped, the copy goes on the outside with your Texas Tower authorization form. DS-64 Lost or Stolen Passport Statement
Include 2 passport quality photos. We recommend that you have these pictures taken by a facility
familiar with passport photo requirements such as Walgreens, CVS, or Kinko's
Proof of U.S. citizenship One of the following :
Original U.S. issued birth certificate
Original U.S. Naturalization Document
An expired U.S. passport that was issued for 10 years
Proof of Identification Make a clear copy front and back of a state-issued or federal-issued I.D. Examples can be a valid state drivers license, military I.D., or government I.D. Must be issued more than 6 months ago.
Flight itinerary or company letter requesting rush service. Sample Letter
Authorization for Texas Tower to act on your behalf . . Please place one inside the sealed envelope and leave one out for us. Authorization letters
Take your documents to your local post office to be witnessed, signed, and sealed. Do not open this envelope once sealed.

This letter will explain in detail how your passport became damaged and why you are requesting a new passport. Our agents can help you with your letter.
