How to Get a Child Passport in Houston with Texas Tower


To travel, you require a valid US passport, and children ages 15 and under require a separate child passport. Child Passport Steps Information
Complete and print the passport application online at the Department of State. Do not sign. Department of State’s passport portal.
Include 2 passport-quality photos. We recommend that you have these pictures taken by a facility
familiar with passport photo requirements such as Walgreens, CVS, or Kinko’s
Proof of Identity Original, Certified birth certificate
Payment to Department of State Check in the amount of $140 payable to Department of State.
One copy of mother and father’s State issued ID. If one of the parents cannot be
present at the time of execution of the documents then a notarized Form DS-3053 is
needed.

C. Travel itinerary Sample Letter
2 copies of the Authorization for Texas Tower to act on your behalf . . Please place one inside the sealed envelope and leave one out for us. Authorization letters
Take your documents to your local post office to be witnessed, signed, and sealed. Do not open this envelope once sealed.
